For business submitted, we require the following information:

1. Complete name, address, and federal tax number of producer
2. ACCORD applications
3. Narrative of operations
4. Information concerning construction and protection of risk
5. 5 year hard copy loss runs
6. Distribution of property values
7. Diagram or map of plant premises if available
8. Photos of plant premises if available
9. Complete driver information if applicable
